Vaccination Records
UW–Madison requires students to share their vaccination status to more quickly prevent the potential spread of these common communicable diseases:
- Measles, Mumps, Rubella (MMR)
- Tetanus, Diphtheria, Pertussis (Tdap)
- Varicella (Chicken Pox)
- Meningococcal (ACWY, B)
- Hepatitis B
To comply with this requirement, students must upload their vaccination records or submit a confirmation form that indicates they are unvaccinated through MyUHS.
Disclosure of vaccination status helps University Health Services best serve each student’s individual health needs and identify and collect specific information related to campus communicable disease prevention.
Students who do not comply with this requirement are subject to a registration hold.

- Incoming students for the summer and fall 2026 semesters must share their vaccination status by August 7, 2026.

How to share your vaccination status
If you are vaccinated or have proof of immunity
Submit your vaccination records online through MyUHS.
- Log in to MyUHS using your NetID and password.
- Click on “Medical Clearances” in the left sidebar.
- Under “Items Required for Clearance” at the top of the page, upload your vaccination records in the “Immunization Records” section.
- Your status will be updated to “Compliant” and display a green check mark once your record is received.
Easily locate your vaccination records: Students from Wisconsin and from states that provide vaccination records to Wisconsin can share their vaccination records from the Wisconsin Immunization Registry (WIR) through MyUHS. In the blue “Check Your Immunization Records” box at the top of the “Medical Clearances” page, click on “Change my response” to opt in to importing your WIR records into MyUHS.
If you are not vaccinated or do not have proof of immunity
Submit an attestation form through MyUHS.
- Log in to MyUHS using your NetID and password.
- Click on “Medical Clearances” in the left sidebar.
- Scroll to the bottom of the page and locate the “Attestations and Declinations” section.
- Click the blue button that reads “Submit an Attestation or Declination.”
- From the drop-down menu in the pop-up box, select “Immunization record – I do not have vaccination records to submit.”
- Type your name and the date, and submit the form.
- Your status in the “Immunization Record” section will be updated to “Compliant” and display a green check mark once your attestation is received.
Students who attest that they are not vaccinated may be subject to additional public health measures in the case of a communicable disease outbreak on campus. This could include quarantine, isolation or otherwise limiting their access to campus facilities, services, classrooms, events and campus jobs.
Vaccination records
A vaccination record is any document from a medical provider written in English that contains the patient’s full vaccination history. If you do not have a record of a vaccine, proof of immunity (such as titers or serology) can also satisfy certain items.
The following types of vaccination records are preferred and are almost always accepted without follow-up:
- Official immunization record from a:
- Healthcare provider (doctor’s office, clinic, hospital)
- State or regional Immunization Information System (IIS)
- Electronic Medical Record (EMR)/Patient Portal screenshot or PDF; must clearly show:
- Student/patient name
- Vaccine name
- Date(s) administered
- Provider or clinic name
- School immunization records
- From a prior college, university, or K-12 school
- Military immunization record
- DD Form 2766C or equivalent
- International immunization record
- Translated to English if not originally in English
If the preferred forms of documentation are not available, the following documents are usually accepted if clearly legible and complete:
- Signed or stamped by the provider (if applicable)
- Pharmacy vaccination record (CVS, Walgreens, Walmart, etc.)
- Public health department vaccination record
- Official vaccination card
- Example: CDC, COVID-19 vaccination card
- Must include name, dates, and vaccine product
- Printed lab or clinic summary
- Signed or stamped by the provider (if applicable)
Locating your vaccination records
If you don’t have access to your vaccination records, or you aren’t sure if you have received a vaccination, you can look up your records in a state immunization registry. Each state has a registry where you can find and download your records. You will need to provide your name, date of birth, and social security number to locate your records in the registry.
You can also contact your primary care provider to obtain your records. An immunization certificate is available for download from the MedProctor portal for your provider to complete.
If you were vaccinated in the United States
- If you are from Wisconsin, you can check the Wisconsin Immunization Registry for your vaccination records.
- If you are from another U.S. state, navigate to your state’s immunization website using this portal.

Contact your original provider for a copy of your records.
If your records are not in English
Vaccination records submitted to MedProctor must be provided in English. Records must be translated by your medical provider or a certified translation service. Students seeking translation for their vaccination records have a few options:
- Contact your medical provider: Request records printed in English from your original medical provider.
- Seek translation support: US Language Services or similar services can assist you with receiving a certified translation of your records. Fees for these services vary.

This requirement services two main purposes:
- It helps us support individual students’ health by ensuring faster, more specific support for students who are at a higher risk when exposed to communicable diseases.
- Access to this data gives public health officials a better understanding of immunity among our campus community.
This is not a requirement of vaccination—it is a requirement to share vaccination status for certain vaccines. Students must share their vaccination records or submit a confirmation that they are unvaccinated (no explanation required).
Please note that specific programs, activities, and employment opportunities on campus may have vaccination requirements.
The information students provide on their vaccination status is confidential and will be retained by UHS as part of a student’s medical record. Learn more about privacy and patient rights.
The information, like the rest of student data, is protected by the federal Family Educational Rights and Privacy Act (FERPA) and will not be disclosed for any reason.
You must complete a brief attestation form online through MyUHS. UW-Madison accepts titers or antibody test results as a measure of immunity for Measles, Mumps and Rubella Varicella, and Hepatitis B. Results for these titers or antibody tests should be submitted into MyUHS using the same process as immunization records. UHS provides antibody titer testing services for a fee. This service is also available through your primary care provider.
If you have some but not all vaccinations, please upload the records that you do have into MyUHS. This will ensure that you are compliant with this requirement.
You may consider speaking with your health care provider on whether any vaccines are recommended based on your personal situation. The UW–Madison policy is not a requirement of vaccination, and you are not required to receive these vaccines.

Once you submit your vaccination records or attestation form, you are compliant with the vaccination disclosure process. Your status in the “Immunization Record” section will immediately be updated to “Compliant” and display a green check mark once your records or attestation are received.
Records will be reviewed by UHS once submitted. Your immunization history in MyUHS will be updated to reflect the records received.
Once you submit your vaccination records or attestation form, you are compliant with the vaccination disclosure process. Your status in the “Immunization Record” section will immediately be updated to “Compliant” and display a green check mark once your records or attestation are received.
If your program or college uses a compliance system—such as ViewPoint Screening—you can log in and download any required records whenever you need them. These records can be uploaded to MyUHS.

Yes. All enrolled UW–Madison students are required to disclose their vaccination status. Even though you are an online student, you may still access other campus services. This requirement covers risks in both classes and beyond. For the purposes of this policy, a student is defined as an individual currently enrolled in a degree-seeking program, even if this program is online.
Your student will need to submit their records online through MyUHS, according to the instructions on this webpage. You will not be able to submit records on your student’s behalf. If you are able to send your student copies of their records, they can upload them via the MyUHS portal.
Due to privacy laws, UHS is unable to share any information about your student’s health records or immunization compliance.
No, you do not need to resubmit your records or attestation. If you submitted vaccination records or an attestation to MedProctor prior to June 15, 2026, this was transferred to MyUHS, and your compliance status will be updated in your MyUHS account.
To confirm whether you are compliant, visit MyUHS, and click on “Medical Clearances” in the left sidebar. Check for a green checkmark that reads “Compliant” next to “Immunization Record.” The details will read “MedProctor Transfer” or “MP Attestation.”
