Diversity, Equity, and Inclusion Resources for Health & Wellbeing Staff

Initiatives that promote diversity, equity, and inclusion are critical to the success and well-being of students and creating a culture of trust and belonging among staff which improves job satisfaction and employee engagement

It is essential that we are proactive in creating a work environment where everyone feels welcome to show up as their full selves and can reach their full potential. We are aware that disagreements, conflicts, and unfair treatment will have a negative impact on employees’ experience and feelings of belonging and inclusion

Retaliation

Retaliation against any individual who, in good faith, seeks resources and assistance through a bias incident resolution process is expressly prohibited. Retaliation is defined as any attempt to coerce, intimidate, interfere, or engage in acts of reprisal against anyone who in good faith seeks resources and assistance or participates in a resolution process.

Employee Consultation Support Form

The Employee Support Consultation form provides supportive resources to employees such as providing advocacy and/or guidance on addressing topics or navigating challenges related to identity/identities in the workplace.

Please note: You are asked to provide your contact information in the form so that you can be contacted for a consultation.

Go to Form

This is an accordion element with a series of buttons that open and close related content panels.

Is this a confidential form?

No. This is a private form, meaning that submissions will only be viewed by April Scott, Director of Diversity & Inclusion, or Jamie Temple, Assistant Director of Trauma-Informed Culture & Organizational Wellbeing. However, because the Director of Diversity & Inclusion is a Responsible Employee, they are mandated to report violations of UW’s Sexual Harassment and Sexual Violence Policy (i.e. sexual harassment, sexual assault, dating/domestic violence, stalking, sexual exploitation and related retaliation).

Reviewers of the form will do their best to not disclose information discussed or reported, but depending on the circumstances, some information may need to be shared with others such as a department director, executive director, or human resources. If information has to be shared, Reviewers will work with the employee to inform them on what will be shared and with whom and also collaborate with the employee on how the information is shared (unless there is a legal mandate in place) in a way that prioritizes the needs of the employee.

Will my submission be shared with my supervisor?

We can work with you so that your concerns are heard but your identity remains unknown to members of your department (unless the concern mandates sharing information via UW policy or your department size is too small where anonymity may not be possible.)

If it is determined that a supervisor needs to be involved, we will work with the employee to determine what is shared. There are times the Reviewer will need to share trends about the overall culture in a department. For example, if there are 25 reports of racist incidents in a single department, the Reviewer may approach the department about addressing the culture of the department in general without ever bringing up your submission.

I fear my supervisor or members of my team may find out about my consultation request and will result in retaliation. How will I be protected against retaliation?

We will hold information sessions with department managers to remind them of the University’s retaliation policy that such behavior is prohibited and how to respond to feedback using trauma-informed principles.

When will I receive a response to my request?

Please allow three business days to receive a response.

How do I know if this is the correct resource for me?

The Employee Consultation Request form is meant to be a resource for employees who are seeking support, advocacy, and training/education on topics related to diversity, equity, inclusion, and/or psychological safety. If your needs exceed the scope of this resource, please contact your human resources representative. If you’d like to consult with someone outside of your organization, you may contact the Ombuds Office or the Office of Equity, Inclusion and Employee Wellbeing.

Who can use this form? Can student staff use the form?

This form can be used by anyone employed by University Health Services and Rec Well, including student staff, interns and/or trainees.
