Biological Safety Cabinet (BSC) and Clean Air Device (CAD) Services Request
Biohazard containment testing and maintenance services are provided (for a fee) for UW research and clinical departments.
Scheduling a Certification
For Biological Safety Cabinets (BSC’s) and Clean Air Devices (CAD’s) in our current database:
Our office will automatically send out a scheduling notification to the lab’s Principal Investigator. Included in this notification packet is a hard-copy reminder that should be posted on the cabinet indicating the date of the certification. To help prevent contamination of your materials and equipment, and to protect our service staff from exposure to hazardous materials, please see that the following actions are taken:
- Notify all biological safety cabinet users of the date of service
- Remove all materials and equipment from the work zone prior to the scheduled service
- Wipe down all work surfaces with an appropriate surface disinfectant.
*A $25.00 charge may be assessed if the BSC is not fully emptied at time of certification.
Our office maintains a file of the certification reports for each BSC on campus. Please let us know if you would like copies of any reports.
You must submit a request for an initial certification of a new BSC or CAD (whether it is new, or new to campus).
Prior to Certification
Please download and complete this form prior to your scheduled service. Forms may be faxed (608-265-0569) or emailed to Marty Fiess prior to service or they can be submitted to the technician at the time of the service.
Canceling or rescheduling a certification
Authorization of our service on the date scheduled in your notification packet is implied unless our office receives notification of cancellation or *rescheduling.
To cancel a certification, notify our office of the cancellation and submit a waiver request with the Office of Biological Safety at biosafety@fpm.wisc.edu
Note: the service will be canceled only if written approval has been provided to us from the Office of Biological Safety prior to the service date.
To reschedule a certification, submit a request. *A $90 fee will be charged for rescheduling out of sequence.
Moving a Cabinet
A biological safety cabinet should not be moved or disassembled without prior approval from the Office of Biological Safety. Most cabinets require gas decontamination prior to relocation. Please fill out this form to notify our office of plans to relocate your cabinet.
Repairs
We keep most replacement parts in stock to provide you with an on-campus repair service. Submit your repair request to our office. A technician will service your BSC as soon as possible.
