New Student Information

What Should I Bring to Campus?

  • Health insurance card (and prescription medication card, if separate)
  • Names and phone numbers of home primary care and specialty care providers
  • Prescription medicines and refill information
  • Thermometer
  • Pain reliever: ibuprofen and/or naproxen
  • Fever reducer: acetaminophen
  • Throat lozenges
  • Antihistamine
  • Alcohol-based hand sanitizer
  • Basic first-aid supplies: adhesive bandages (ex: Band-Aids), antibacterial ointment, hydrocortisone cream

Appointments

MyUHS

Activate your MyUHS account by visiting http://www.uhs.wisc.edu/appointments/myuhs/

  1. Click on the gold button: "Log in to MyUHS."
  2. Click on the link: "Sign up for an account."
  3. Enter your UW–Madison student ID, birth date, and last name.
  4. Follow the instructions to activate your account.
  5. If you experience problems, email myuhs@uhs.wisc.edu.

Before the semester begins, you can use MyUHS to fill out your Immunization and Health History Form. After the semester begins, you will also become eligible to make online appointments and exchange secure messages with your providers.

Services

Immunizations

There is no immunization requirement to enter UW–Madison, but UHS strongly recommends that all students be up to date on certain immunizations before coming to campus. Immunizations are also available at UHS, for a fee.

Immunization and Health History Form

You can download PDFs of the four-part Immunization and Health History Form and start gathering the information you’ll need to fill it out while you make your appointments for immunizations and wait to be able to sign up for MyUHS.

Requirement for students in residence halls

Students living in residence halls are required by Wisconsin statute to report whether they have or have not received immunizations for hepatitis B and meningococcal disease. You can fulfill this requirement by completing your Immunization and Health History Form.
